What is critical illness cover?

Critical illness cover (CIC) pays out a tax-free lump sum if you're diagnosed with one of a specified list of serious medical conditions — such as cancer, heart attack, or stroke. Unlike life insurance, you receive the payout while alive. Unlike income protection, it's a one-off payment rather than ongoing income.

The payout is yours to use however you need — paying off your mortgage, adapting your home, covering treatment costs, or simply giving you financial breathing space during recovery.

What conditions are typically covered?

Most policies cover a "core" set of around 40–50 conditions, including:

  • Cancer (most types, at specified severity)
  • Heart attack
  • Stroke
  • Multiple sclerosis
  • Major organ transplant
  • Kidney failure
  • Total permanent disability
  • Loss of sight, speech, or hearing
  • Parkinson's disease (early onset)

Many policies now offer additional "enhanced" definitions and a wider list of conditions — sometimes 80+. The detail matters: policies vary significantly in what they cover and how they define each condition.

💡 Around 1 in 2 people in the UK will be diagnosed with cancer in their lifetime. Critical illness cover exists precisely for these moments.

What's NOT covered

Critical illness cover is not a catch-all. Key exclusions typically include:

  • Pre-existing conditions (conditions you had before the policy started)
  • Conditions not on the insurer's specific list
  • Early-stage cancers below a defined severity threshold
  • Alcohol or drug-related illness
  • Self-inflicted injury

Reading the policy definitions carefully — ideally with a protection adviser — is essential. Two policies at similar premiums can cover very different things.

How much cover do I need?

Common approaches:

  • Mortgage cover: Match the cover amount to your outstanding mortgage, so it could be paid off if you're seriously ill
  • Income replacement: 2–5 years of salary as a lump sum to fund recovery
  • Specific needs: Cost of private treatment, home adaptations, or childcare during recovery

A common starting point is 3–5× annual salary, but the right amount depends on your mortgage, savings, income protection cover, and family circumstances.

Is it worth it?

Critical illness cover is more expensive than term life insurance, and many people wonder whether it justifies the cost. The case for it:

  • You're statistically more likely to be diagnosed with a critical illness than to die during working age
  • Treatment and recovery can be very expensive, especially if you need private care
  • A lump sum gives you choices that income replacement alone doesn't

For families with mortgages and children, critical illness cover is often the product that makes the biggest financial difference at the worst possible time.
